Does not come assembled - you must assemble it, and once you do, it leans a little forward due to the hanging screws being on the top but on the bottom. It looks as shown, although dimensions are slightly different than indicated. It is 12" wide x 5" deep, and I bought the one that said it was 18" wide by 4" deep.
